The FOPH has published its circular on the 2026 triennial review of the list of specialties. Much of the content is familiar, with only a few new additions. One notable change is a clarification with a noticeable effect: TQV as 5-year therapy costs for dose adjustment.

What exactly does the circular say?

• New: When adjusting the dose, the TQV is calculated over five years.
• Prerequisite: Long-term therapy ≥ 5 years; dose adjustment not applicable to all preparations.
• Oncology: Treatment duration can be determined based on progression/median duration.
• Practical: Titration/charging phases are discussed in a "smoothed" manner rather than being driven by daily costs.
