Stop rules as enforcement levers

Nexpovio (new to the specialty list as of February 1, 2026) shows how stop rules are evolving in practice: they no longer stand alone, but are becoming part of an "enforcement architecture." What is new is not so much the formula "reimbursement until progression" itself, but rather the interlinking with medical examiners, objective criteria, and a refund per package—this makes the limitation enforceable and budgetable for the FOPH.
Key points of the limitation:
- Stop rule is implemented with cost approval and medical examiner (enforcement hardship)
- Additional objective criteria (IMWG, ECOG ≤ 2, organ function) reduce room for interpretation
- "Remuneration up to progression" serves as an exposure brake: who is remunerated and for how long can be clearly defined.
